We also provide SKILLED HOME HEALTH CARE because sometimes you need skilled nursing. For those times when a loved one has been hospitalized and needing care after hospitalization. We are here for you. From physical therapy to in-home support after surgery, or even terminal illness, we are there.  OUR SKILLED CARE PROFESSIONAL SERVICES are provided by nurses (RN or LPN), physical therapist (PT), occupational therapist (OT), or speech therapist (ST). Our team is there to carry out a plan of care ordered by a physician. When your loved one is released from the hospital, we will be there. We can help with discharges, help families to facilitate the return home and help to prevent re-admissions to the hospital by providing ongoing care.
